AG Medical Practice – Terms and Conditions

Effective Date: 26 January 2026

1. Appointments

Appointments should be booked in advance and are subject to availability.

Patients are requested to arrive on time. Late arrival may result in a shortened consultation or rescheduling.

The practice reserves the right to prioritise emergency cases.

2. Cancellations and Missed Appointments

Patients are requested to cancel appointments at least 24 hours in advance.

Failure to attend a scheduled appointment without notice may result in a missed appointment fee.

3. Fees and Payments

Consultation fees are payable at the time of the visit unless otherwise agreed.

Follow-up appointments will be billed as full consultations unless the patient returns within 7 days of consultation for the same condition.

Fees may vary depending on the length and type of consultation or procedure.

Medical aid claims remain the responsibility of the patient unless the practice offers direct claims.

4. Medical Aid and Billing

Patients are responsible for providing accurate medical aid details.

The practice is not liable for claims rejected by medical aid schemes.

5. Confidentiality and Patient Records

All patient information is treated as confidential and managed in accordance with applicable privacy and health laws.

Medical records may be released only with written patient consent or when legally required.

6. Consent to Treatment

By attending the practice, patients consent to medical assessment and treatment deemed necessary by the practitioner.

Additional consent may be required for certain procedures.

7. Prescriptions and Referrals

Prescriptions are issued at the doctor’s discretion and in line with medical guidelines.

Referrals to specialists or other healthcare providers are made when clinically appropriate.

8. Test Results and Follow-Up

Patients are responsible for following up on test results unless otherwise advised.

Results may require a follow-up consultation.

9. Respectful Conduct

The practice maintains a zero-tolerance policy for abusive, aggressive, or inappropriate behaviour towards staff or other patients.

The practice reserves the right to refuse service in such circumstances.

10. Limitation of Liability

Medical care is provided according to accepted professional standards.

The practice is not liable for outcomes resulting from incomplete or inaccurate information provided by the patient.

11. Changes to Terms and Conditions

The practice reserves the right to update these terms and conditions at any time.

Updated terms will be made available to patients upon request or displayed at the practice.

12. POPIA, HPCSA and Patient Health Charter

Copies of these documents can be provided upon request by any enquiring patient.
